You have been selected for the 2013 Corpus Christi IceRays Midwest Camp to be held June 21st - 23rd at the Fond du Lac Blue Line in Fond du Lac, WI.

Your selection into 2013 Corpus Christi IceRays Midwest Camp has been based on one or a combination of the following recruiting procedures:

- Our coaching or scouting staff recommendation
- Your coach’s recommendation
- Follow up scouting by a member of our staff
- Camp invitation

Please see the enclosed documents pertaining to the camp itinerary and hotel information.

A confirmation will be emailed to you upon receipt of your registration form. Please delays booking the logistics for your travel and accommodations until you have received a confirmation email for the 2013 Corpus Christi IceRays Midwest Camp. 

Every player must have USA Hockey registration that is up to date prior to the camp and be able to present a copy prior to skating. All forms and fees must be received promptly as we will be limiting the amount of players in Camp.  Please contact jworlton@goicerays.com or call the team office at 361-814-7825 with any questions you may have. USA HOCKEY REGISTRATION 

Your USA Hockey Registration # must be entered in your registration or brought with you to the Camp.

-  Updated 2012-2013 USA Hockey Individual Membership Registration #, which can
    be completed online at www.usahockey.com.  *Please note that even if your current
    registration shows that it is valid through 2012, you must still re-register prior to
    camp so you are valid for the 2012-2013 season.

Please bring the following with you to Midwest CAMP / Or fill out on site

1. Upon arrival you will be asked to fill out a “Consent to Treat” form.  Please note that if you are under 18 years of age, a parent / guardian signature is required and may be requested prior to camp attendance.  jworlton@goicerays.com

2012-2013 USA Hockey Individual Membership Registration (IMR)

• Go to www.usahockey.com
• Click on Membership and then click register online
• Click “Ice Players & Coaches”
• Fill out required information

*If player is under the age of 18, a parent or guardian must fill out on player’s behalf

HOTEL ACCOMODATIONS

The hotel listed below is centrally located in Fond du Lac area and within close distance from the Arena.  If you chose to stay at any other location, please be advised to find transportation to and from the Arena.  The IceRays are not responsible for the players travel to and from the Arena, however several hotel services are willing to provide shuttle services.
